Terms & Conditions form4 – Platform Terms of Use

1Member Eligibility

The form4 platform is intended exclusively for users who meet the following eligibility criteria. form4 reserves the right to review any member's eligibility at any time and take appropriate action if the requirements are not met.

• You must be at least 18 years of age at the time of registration and throughout the duration of your use of the form4 platform.
• You must be a resident of Malaysia or are a valid Malaysian citizen.
• You must have full legal capacity to enter into a binding agreement under Malaysian law.
• You are not currently enrolled in any active self-exclusion programme, whether with form4 or any other platform.
• You are not prohibited by any law, regulation, or court order from using the online gaming platform.

2Account Registration

Each individual is permitted to hold only one (1) form4 account only. Holding multiple accounts is strictly prohibited and may result in the closure of all associated accounts without any compensation.

During the registration process at form4, you are required to provide accurate, complete, and up-to-date information, including:

• Full name as per your Malaysian identity card (MyKad)
• A valid, active Malaysian phone number
• An active, reachable email address
• A Malaysian bank account or eWallet details for transaction purposes

You are solely responsible for keeping your form4 account password confidential. form4 will never ask for your password through any communication channel. If you suspect your account has been accessed without authorisation, contact the form4 support team immediately.

3Deposits & Withdrawals

Form4 supports a wide range of local Malaysian payment methods for members' convenience. All transactions are processed exclusively in Malaysian Ringgit (MYR).

The minimum deposit on the form4 platform is RM30 per transaction. The maximum daily deposit limit depends on your account tier and the payment method used. Deposits are typically completed in under one minute.

For withdrawals, form4 members must ensure their account has completed the full identity verification (KYC) process. The minimum withdrawal amount is RM50 per request. Processing times range from 3 to 24 hours depending on the chosen method. No withdrawal fees applied to fully verified accounts.

4Bonus & Promotions

Form4 offers a variety of bonuses and promotions to both new and existing members from time to time. Each promotion carries its own specific terms and conditions, which should be read carefully before redemption.

• Welcome Bonus: Available to new members making their first deposit. Wagering requirements will be clearly stated in the relevant promotion terms.
• Weekly Cashback: Calculated based on net losses over a one-week period. Cashback rates and maximum caps vary depending on the current promotion.
• Loyalty Programme: Active form4 members earn reward points that can be redeemed for gaming credits. These points cannot be exchanged directly for cash.

5Game Rules

All games available on the form4 platform – including video slots, Live Casino, video poker, and table games – are subject to their respective game rules. Game outcomes are final and may not be disputed unless there is substantial evidence of a technical system failure.

• All games on form4 use a Random Number Generator (RNG) that has been independently verified by an accredited third party.
• In the event of a technical fault or connection disruption during a game session, form4 reserves the right to void the affected round and return the wagered amount to the member's account balance.
• Any attempt to hack, manipulate, or interfere with form4's gaming systems constitutes a criminal offence and will be reported to the relevant authorities.
• The minimum and maximum betting limits for each game will be displayed within the respective game interface.

Form4 reserves the right to void or reverse any winnings obtained through technical vulnerabilities, system errors, or any unauthorised means. form4's decision in such matters is final.

6Member Conduct

All form4 members are expected to conduct themselves in a respectful and responsible manner while using the platform. The following behaviours are strictly prohibited and may result in account suspension or closure:

• Using automated software, bots, or any third-party tools to play or make decisions within games
• Colluding with other members to gain an unfair advantage in multiplayer games
• Posting threatening, abusive, or violent content on any form4 communication channel
• Using the form4 platform for money laundering or any other criminal activity
• Registering using another person's identity or personal information without their consent
• Channelling funds through a form4 account without genuine intent to play

Form4 works closely with the relevant authorities and reserves the right to report any suspected unlawful activity to the appropriate enforcement agencies in Malaysia.

7Account Closure

You may close your form4 account at any time by contacting our support team. Prior to closure, please ensure all available funds have been withdrawn, as form4 is not responsible for any unclaimed balances after 90 days from the account closure date.

Form4 also reserves the right to close or suspend a member's account in the following situations:

• Violation of any part of these Terms & Conditions
• Identity fraud or providing false information during registration
• Activities suspected of being related to money laundering
• Requests from legitimate legal authorities
• Self-exclusion requests submitted by the member themselves
• Account inactivity for more than 12 consecutive months

8Privacy & Data Protection

Form4 takes every member's privacy seriously. All personal data you share with form4 will be used solely for platform operations, identity verification, payment processing, and service communications.

9Limitation of Liability

Form4 shall not be liable for any indirect, consequential, or special losses arising from the use of this platform, including but not limited to:

• Service disruptions due to scheduled or unscheduled system maintenance
• Losses due to internet connection disruptions on the member's end
• Any damage to a member's device arising from use of the platform

Form4's total liability to any member, under any circumstances, shall not exceed the total amount deposited by that member in the 30 days preceding the relevant claim.

10Amendments to Terms & Conditions

Form4 reserves the right to amend, update, or improve these Terms & Conditions at any time without mandatory prior notice. However, form4 endeavours to inform members of material changes via email or in-platform notifications at least 14 days before the changes take effect.

Your continued use of the form4 platform following any amendments to these terms constitutes your acceptance of the updated terms. We therefore encourage you to review this page periodically to stay informed of the latest applicable terms.

If you disagree with any amendments made, you have the right to close your account and withdraw all available funds. Please contact the form4 support team for assistance with this process.

These Terms & Conditions are governed by and construed in accordance with the laws of Malaysia. Any disputes arising shall be resolved through mutually agreed dispute resolution mechanisms or a court of competent jurisdiction in Malaysia.
